More than 1 in 10 Americans has some form of diabetes. This metabolic condition disrupts the body's regulation of glucose in the bloodstream, giving rise to health problems like slow wound healing, frequent infections and blindness—and it's the seventh leading cause of death in the U.S. It can affect every aspect of your health including your teeth and gums.
Fortunately, people with diabetes can manage it through medication, diet and exercise. Even so, the disease could still have a profound effect on physical health. The mouth in particular becomes more susceptible to a number of oral conditions with diabetes.
In recognition of American Diabetes Month in November, here's how diabetes could put your oral health at risk for other diseases and what you can do about it.
Gum disease. Diabetics are at high risk for severe periodontal (gum) disease because of a characteristic shared by both conditions: inflammation. What is normally a healing response of the body to infection or trauma becomes destructive if it becomes chronic. Studies show that, due to their inflammation connection, diabetes can worsen gum disease, and gum disease can make it harder to bring diabetes under control.
Dry mouth. Chronic dry mouth is another possible consequence of diabetes that harms oral health. It's the result of the body not producing enough saliva. Because saliva supplies antigens to fight infection and neutralizes oral acid, which erodes tooth enamel, inadequate saliva increases the risk of both tooth decay and gum disease.
Thrush. Also known as oral candidiasis, thrush occurs when the fungus Candida albicans spreads along the inside surface of the mouth. This fungal infection can produce painful white lesions that make it difficult to eat or swallow. Complications from diabetes, including dry mouth and raised glucose levels in saliva, increase a diabetic's chances of developing thrush.
Implant complications. An implant's stability depends on the healing period after implant surgery when bone cells grow and adhere to its titanium surface. But because diabetics can experience slow wound healing, the bone may not fully develop around the implant and eventually causing it to fail. Fortunately, this is less of a problem if the patient has their diabetes under control.
So, what can you or someone you love with diabetes do to avoid these oral health pitfalls? For one, practicing daily brushing and flossing—and seeing your dentist on a regular basis—is paramount for reducing the risk of any dental disease. Additionally for diabetics, consistently keeping your condition under control will likewise lessen the impact it may have on your teeth and gums.
In addition to the usual tooth and gum problems, dentists also see patients with soft tissue infections in and around the mouth. One of the more common of these is the irritation or "cracking" of the corners of the mouth.
Formally known as angular cheilitis (or perleche, a French word, meaning "to lick"), cracked mouth corners are localized irritations made worse by saliva accumulation or an accompanying yeast infection. They're prominent among children and young adults who drool during sleep or while wearing orthodontic braces.
Older adults can also develop cracked mouth corners because of deep wrinkle lines around the mouth ("marionette lines") or tissue irritation from wearing dentures. Teeth loss, especially in the back of the jaws, can weaken facial support leading to collapse of the bite, which can contribute to angular cheilitis.
The condition can cause anything from minor discomfort at the mouth corners to a yeast infection that spreads throughout the mouth and throat. Whatever the symptoms, treatment usually begins with antifungal medication in the form of a mouthrinse or a topical ointment. The dentist may also prescribe a steroid ointment like zinc oxide paste to control inflammation and serve as a barrier against infection.
If the infection has spread beyond the mouth corners, patients may also need to use an antibacterial mouthrinse (usually chlorhexidine) to clear up the infection and help prevent a relapse. Besides cleaning their appliances with chlorhexidine, denture wearers with angular cheilitis should also take their dentures out at night to reduce the chances of a reoccurrence.
Along the same vein, patients who contend with frequent cracked mouth corners and who have missing teeth should have those teeth replaced by some form of restoration. If that involves dentures, it's important to maintain a good fit with them to reduce the chances of tissue irritation. And patients with deep wrinkle lines around their mouth may be able to lessen them through dermatological treatment.
Even though cracked mouth corners rarely pose a major health problem, the discomfort they cause can be a drag on your daily life and activities. Remember that you don't have to suffer—a visit to your dentist could start you on your journey toward relief from this irritating problem.

We Americans spend billions on oral hygiene products each year, primarily to fight tooth decay and gum disease. But there's also a secondary motive—to freshen our breath. Our obsession with our breath's olfactory quality has even given rise to National Fresh Breath Day on August 6.
Bad breath is usually not a serious health issue, but it can be a big deal in other respects. In a recent Match.com survey, more than a third of its 5,000 respondents said fresh breath was their top concern during a date.
Romance aside, bad breath can also adversely affect other social and career relationships. Given that, it's no wonder we buy mints and mouthwashes by the ton. Unfortunately, much of what people purchase only masks breath odor without addressing the underlying cause.
While serious conditions like diabetes, liver disease or cancer can give rise to it, the primary source of bad breath is oral bacteria. Many of the hundreds of bacterial strains inhabiting our mouths generate volatile sulfur compounds (VSCs) with a smell akin to rotten eggs or decaying animal or vegetable matter. The longer these bacteria remain in the mouth, the more VSCs and their unpleasant odors they create.
Bacteria mainly thrive in a thin film of food particles known as dental plaque. It can build up quickly on tooth and gum surfaces, particularly with ineffective or non-existent oral hygiene. Dental plaque also causes tooth decay or gum disease, which may also contribute to unpleasant mouth odors.
Chronic dry mouth, in which the body isn't producing enough saliva, also encourages bacterial growth. Among its many functions, saliva's antibacterial compounds reduce bacterial populations. Without sufficient saliva, though, VSC-producing bacteria can run amok.
There's nothing wrong with swishing some mouthwash or popping a breath mint before a big meeting. But if you really want to alleviate bad breath, it's better to take direct action against the oral bacteria causing it.
The best thing you can do is maintain a daily regimen of brushing and flossing to remove dental plaque. This should also include cleaning the top surface of your tongue, which can be a prime hiding place for bacteria, particularly at the back of the tongue. You can simply brush your tongue with your toothbrush or use a special tongue scraper.
You should also keep up regular dental visits, which include cleanings to remove any residual plaque and tartar (hardened plaque). You should also see the dentist to treat any occurring dental diseases or conditions, including chronic dry mouth.
Fresh breath has everything to do with a clean, healthy mouth. Keeping it so can help you avoid those embarrassing odors.

The mouth is a crowded place with nerves, blood vessels and sinus cavities sharing common space with the teeth and gums. Although important in their own right, these structures can also hinder treatment for complex dental situations like dental implant surgery or impacted teeth.
Treating these and similar situations depends on getting an accurate depiction of “what lies beneath.” Conventional x-rays help, but their two-dimensional images don't always give the full picture. There's another way—cone beam computed tomography (CBCT).
Similar to CT scanning, CBCT uses x-ray energy to take hundreds of “sliced” images that are then re-assembled with special software to create a three-dimensional model viewable on a computer screen. CBCT is different, though, in that it employs a scanning device that revolves around a patient's head, which emits a cone-shaped beam of x-rays to capture the images.
A dentist can manipulate the resulting 3-D model on screen to study revealed oral structures from various angles to pinpoint potential obstacles like nerves or blood vessels. The detailed model may also aid in uncovering the underlying causes of a jaw joint disorder or sleep apnea.
CT technology isn't the only advanced imaging system used in healthcare. Another is magnetic resonance imaging (MRI), which excites hydrogen atoms in water molecules. This produces different vibration rates in individual tissue structures, which are then translated into detailed images of these structures. Unlike CT or CBCT, MRI doesn't use x-ray energy, but rather a magnetic field and radio waves to produce the atomic vibrations.
But while providing good detail of soft tissues, MRI imaging doesn't perform as well as CBCT with harder tissues like bone or teeth. As to the potential risks of CBCT involving x-ray radiation exposure, dentists follow much the same safety protocols as they do with conventional x-rays. As such, they utilize CBCT only when the benefits far outweigh the potential x-ray exposure risks.
And, CBCT won't be replacing conventional x-rays any time soon—the older technology is often the more practical diagnostic tool for less invasive dental situations. But when a situation requires the most detailed and comprehensive image possible, CBCT can make a big difference.

Your tooth enamel is often under assault from oral acid produced by bacteria and certain foods. Unless neutralized, acid can erode your enamel, and lead to destructive tooth decay.
But there's another type of acid that may be even more destructive—the acid produced in your stomach. Although important for food digestion, stomach acid outside of its normal environment can be destructive. That includes your teeth, if stomach acid finds its way into your mouth. And that can happen if you have gastroesophageal reflux disease (GERD).
GERD, a chronic condition affecting 1 in 5 adults, is caused by the weakening of the lower esophageal sphincter, a ring of muscle at the intersection of the esophagus and the stomach that prevents stomach acid from traveling back into the digestive tract and damaging the esophageal liner.
It's also possible for stomach acid to travel as far up as the mouth. With a pH of 2.0 or less, stomach acid can lower the mouth's normal pH level of 7.0 well below the 5.5 pH threshold for enamel softening and erosion. This can cause your teeth, primarily the inside surfaces of the upper teeth, to become thin, pitted or yellowed. Your teeth's sensitivity may also increase.
If you have GERD, you can take precautions to avoid tooth damage and the extensive dental work that may follow.
- Boost acid buffering by rinsing with water (or a cup of water mixed with a ½ teaspoon of baking soda) or chewing on an antacid tablet.
- Wait about an hour to brush your teeth following a reflux episode so that your saliva has time to neutralize acid and re-mineralize enamel.
- If you have chronic dry mouth, stimulate saliva production by drinking more water, chewing xylitol gum or using a saliva supplement.
You can also seek to minimize GERD by avoiding tobacco and limiting your consumption of alcohol, caffeine or spicy and acidic foods. Your doctor may also prescribe medication to control your GERD symptoms.
Preventing tooth decay or gum disease from the normal occurrences of oral acid is a daily hygiene battle. Don't let GERD-related acid add to the burden.
